Hear ye, hear ye! You are hereby invited to join us at Redemption Hill Church for a week of Vacation Bible School. We’ll spend our week journeying through a royal kingdom so we can learn about the King. Liability and Medical Waiver

I hereby give permission for my child to attend Vacation Bible School at Redemption Hill Church. I waive any and all rights and claims of any nature I may have against Redemption Hill Church for any and all injuries or damages of any nature which may occur as a result of my child's participation in this VBS. I understand that injury or illness to my child may result from or during VBS. In case of a medical emergency, I authorize my child to be given medical or hospital treatment as deemed advisable for their health and well-being. I agree to the release of any records necessary for treatment, referral, billing, or insurance purposes, and acknowledge that I will be responsible for any medical bills incurred by my child at a local hospital or elsewhere. I also affirm that the medical information provided on this form is correct.
